Maximizing potential for people with special needs

Intake and Supports Coordination Management
Monitors the entry into all mental retardation services. Based on intake evaluations, support coordinators work with families to develop individualized service plans for their mentally retarded relatives and assist them in obtaining vocational, socialization and recreational programs through COMHAR or other agencies. Family Driven Support Services encourages families to care for their relatives at home, reducing or eliminating the need for more intensive and restrictive services. Support services, designed by the family and support coordinator, may include financial assistance for needed services; referrals to informal support groups, respite co-ops and information sharing networks; and linkages with other community services, resources and programs. Core Team Services
Provides in-home clinical services to adults with developmental disabilities/mental retardation who live in the community, plus consultation and skills training for their caretakers so these adults can remain in the community and achieve greater independence. Consultants in behavioral intervention, mobility and orientation and sexuality education develop an individualized program for each client, train families and caretakers to carry out the program at home, and help them to handle the challenges of living with and caring for persons with disabilities. Training Services
Serves mentally retarded adults through four programs that use a team approach to support individual needs and interests—from development of basic life skills to employment follow-up services. Vocational Training instills and strengthens work-related skills and behaviors in a facility based "workshop". Community Services helps individuals to find and retain competitive employment. Older Adult Services provide both center based and community integrated services and activities to meet the needs and interests of seniors and their caretakers. Residential Services
COMHAR's community based residences serve individuals with mental retardation who may also have additional disabilities such as cerebral palsy, hearing and vision impairments, physical handicaps and mental illness. Located throughout Philadelphia and in Montgomery County, these homes offer varying levels of supervision and assistance based on the individuals' needs. Residential programs provide staff support and a range of training, habilitation services, and community experiences that enable each person to develop the skills and competencies required for increased independence. Staff encourage individuals to participate actively in their own care and in making life choices. Access to COMHAR's residential services is by referral from Philadelphia's Office of Mental Retardation Services.

Bilingual staff members are available to work with Spanish speaking individuals—both adults and children—in most COMHAR programs.

Access to COMHAR's mental retardation services is through our Director of Supports Coordination, who evaluates the needs of individuals and their families. After the initial assessment, a Supports Manager works with each family to develop an individualized service plan for obtaining appropriate services through COMHAR or other agencies.
